Serious Occurrence Policy

SERIOUS OCCURRENCE PROCEDURES

  • The child shall be provided with immediate medical attention when warranted
  • If warranted, staff will contact the appropriate services such as Fire and Police Services, Child protection agencies etc.
  • The staff or any other person witnessing or having knowledge of the occurrence shall orally report the occurrence immediately to the Executive Director/Supervisor/Designate.
  • The Executive Director/supervisor/designate notified will conduct a preliminary inquiry immediately.
  • All persons having knowledge of the occurrence will remain at the program location until they are excused by the supervisor/designate and/or Executive Director.
  • If, on the basis of the preliminary inquiry, there is reason to suspect that a child has been abused, the staff member in charge will immediately request a medical examination of the child and photographs as might be appropriate. A written report and an opinion as to the probable cause of the injury shall be requested from the attending physician.
  • If, on the basis of preliminary inquiry, there is reason to suspect that there has been abuse of a staff member, the staff member in charge will immediately request a medical examination of the injured staff member.
  • The Executive Director or designate notifies Board of Directors of the incident immediately.
  • The Executive Director or designate will submit the Serious Occurrence through Child Care Licensing System (CCLS) within 24 hours of becoming aware of the incident.
  • If the child is involved with York Region Children’s Services on Fee Assistance a copy of the Serious Occurrence must be sent to the Children’s Services Representative.
  • If allegations are made against the Centre or staff a report must also be sent to the coordinator of Community Services at the York Region District School Board.
  • Where applicable and with possibly consultation with the Ministry, contact police or the Crown Attorney.
  • Any situations that are related to COVID-19 must follow current regulations

ON SITE PROCEDURES

For any accident which occurs at the Centre, an accident form must be filled out by the staff member who witnessed the accident. The staff member who witnessed the accident or a designate is responsible for making the parent(s) aware of the occurrence and ensuring the accident form is signed by the parent(s).

In the event of a major accident where it is judged by the supervisor/designate/Executive Director that a trip to the hospital is required, the following procedure will apply:

  1. The child should be taken by ambulance to the nearest hospital or hospital of preference, depending on the nature of the emergency.
  2. A staff member will go with the child unless the parents arrive before the ambulance leaves.
  3. The child’s emergency file containing the emergency consent form and medical record must accompany the child to the hospital.
  4. The staff should ensure that an Accident Form is signed by the parent(s) as soon as possible and a copy is provided to the parents.

OFF SITE PROCEDURE

For any accident which occurs on a field trip, an Accident Form must be completed by the staff member who witnessed the accident. The staff who witnessed the accident or a designate is responsible for making the parent(s) aware of the occurrence and ensuring that the accident form is signed by the parent(s). 

In the event of a major accident where it is judged by the supervisor/designate/Executive Director that a trip to the hospital is required, the following procedure will apply:

  1. One staff will take the Emergency File Information and go with the child to the nearest hospital by ambulance.
  2. The other staff will gather the children and return to the centre as soon as possible.
  3. Executive Director/supervisor/designate will contact parent(s) to meet the child and the staff at the hospital.
  4. Executive Director/supervisor/designate will meet staff at the hospital with the emergency file.

 ***Staff will not leave the Centre without the “Emergency File Information” for each child.***

 The Executive Director or Designate completes the Report of Serious Occurrence and submits in through CCLS.

NOTIFICATION FORM

Kids Come First is required to post a high-level Serious Occurrence Form at the day nursery when a Serious Occurrence has happened. Following the submission of a Serious Occurrence to the Ministry and within 24 hours of becoming aware of an occurrence as set out under the Serious Occurrence Reporting Procedures, a Notification Form to communicate information to the parents about a serious occurrence that has occurred at the Centre will be posted on the Parent information board near the Child Care License and Licensing Summary Chart.
